What Makes Cellulite Difficult to Treat, and What Are My Options?

Cellulite is a condition characterized by small divots in the skin that create an “orange peel” texture. Though common, it is difficult to treat because its development is influenced by complex interactions of fat distribution, muscle fibers, skin quality, and even hormone levels and genetics. (1)

Cellulite occurs when the fibrous bands beneath the skin pull down on the underlying fat, and this pressure causes the dimple-like divots. Approximately 80-90% of women experience cellulite, typically on areas like the thighs, buttocks, or hips. (1) For many, cellulite is a source of self-consciousness or embarrassment. While there is no cure for this condition, certain procedures can help reduce or disguise the appearance of cellulite.

About Cellulite Reduction

Cellulite reduction refers to a group of procedures that aim to improve the appearance of your skin, creating a smoother look. Cellulite usually begins to appear between the ages of 20 and 30, and it can be a source of embarrassment for those who experience it. The dimpled skin can make it difficult to feel comfortable and confident wearing different types of clothing.

Although connective bands are important for skin structure, they can lead to dimples when they are too tight. (2) Many minimally invasive cellulite treatments work by releasing the connective bands (septa) beneath the skin.

What Factors Contribute to Cellulite Development?

Cellulite is a stubborn condition that is often linked to hormonal changes and genetics, which can affect the distribution of fat cells beneath the skin. Some people are more prone to developing cellulite than others. Hormonal fluctuations, specifically of the female sex hormones, may also affect the structure of the underlying fat, which could contribute to cellulite. Lifestyle factors, such as diet and exercise, smoking, pregnancy, and weight loss, are also known to impact the condition of your skin. These changes might cause cellulite to become more visible.

Aging also results in a loss of collagen and elastin, which can cause skin laxity and loss of firmness, exacerbating the appearance of cellulite. Poor lymphatic drainage and circulation can also play a role, resulting in fluid retention and swelling that accentuates the appearance of cellulite, making it harder to conceal. As women approach menopause, reduced blood flow and lower levels of collagen can also affect skin texture and result in a lumpy appearance. (3)

If you have cellulite, Dr. Friedlander can refer you to trusted specialists who perform cellulite treatments, many of which are FDA-approved. One of the most effective treatments is Aveli, which uses a specialized device to sever the connective bands causing cellulite.

Cellulite Reduction vs. Liposuction

Liposuction removes pockets of excess fat to contour and sculpt a patient’s ideal body. It is not an effective cellulite treatment because it does not release the connective bands beneath the skin—it only addresses fat. Nonetheless, it is an effective method for addressing stubborn fat and enhancing confidence. In 2023, specialists performed more than 347,700 liposuction treatments, transforming the lives of hundreds of thousands of patients. (4)

During a liposuction treatment, Dr. Friedlander administers tumescent fluid–a combination of saline, epinephrine, and lidocaine–which causes the fat cells to swell for easier removal and numbs the surrounding tissue to reduce recovery time. Dr. Friedlander then inserts a VASER liposuction probe, which emits ultrasound waves to gently heat and liquefy fat cells, making them easier to remove with the cannula. This leads to more effective body contouring without damaging any surrounding tissues.

Cellulite vs. Lipedema

Lipedema is a chronic condition that affects approximately 11% of Americans who struggle with fluid buildup in the lower body, resulting in what is commonly referred to as a “tree trunk” appearance. Although the fluid can cause the skin to appear dimpled, this is the result of painful bulges of fat, not cellulite. As lipedema develops, its initial stages are often mistaken for cellulite or weight gain due to fluid retention.

Currently, there is no cure for lipedema, either. However, Dr. Friedlander offers liposuction as a treatment option for reducing lipedema. While not an effective cellulite treatment, liposuction has helped thousands of patients manage their lipedema. Lipedema treatments can reduce chronic pain, increase self-confidence, address fluid buildup, and improve your quality of life through easier mobility and exercise. If you suffer from lipedema, we encourage you to schedule a consultation with Dr. Friedlander to learn how you can address it. FAQ

How long will the results of cellulite reduction last?

When considering cellulite reduction options, it is important to keep in mind that there is no cure for the condition at the moment. After undergoing cellulite reduction treatments, patients often see their cellulite return after a period of time. Cellulite is considered a chronic condition, meaning it persists even after an initial treatment.

Is cellulite reduction painful?

During many cellulite reduction methods, anesthesia is administered to ensure a comfortable procedure. Patients may experience some discomfort afterward, but it is typically considered a tolerable type of procedure.

Can I combine cellulite treatments with other body contouring treatments?

Yes, you can combine cellulite treatments with other body sculpting procedures to enhance your appearance and achieve your desired look. We offer a wide range of body sculpting treatments to help patients achieve their ideal figures and aesthetic goals.

Will I need multiple cellulite treatments?

Because cellulite is a chronic condition, cellulite often recurs even after treatment. So, if you do undergo cellulite reduction treatment, it is likely that you will need to undergo future treatments if you would like to maintain your results.

Is there a permanent treatment for cellulite?

No, there is currently no cure or permanent treatment for cellulite. Cellulite treatments temporarily release the connective bands beneath the skin that pull on excess fat. This provides an improvement in skin appearance, but cellulite often recurs over time.

How long does cellulite treatment take?

This depends on what type of treatment you receive. If only cellulite is being addressed, these kinds of treatments typically take 45 minutes. The length of your procedure will depend on your specific treatment plan and whether you need additional treatments to achieve your aesthetic goals.
